Safety, quality improvement (QI) and service integration

The Safer Salford programme is a system-wide, quality improvement focused initiative that began in 2016 in the wake of the Francis and Berwick reports (2013), and Salford’s involvement in The Health Foundation’s Safety Measurement programmes (2014a, 2014b).

Building capability to deliver personalised care and secure buy-in from disadvantage patient populations

The borough of Knowsley is the fifth most deprived in the country and suffers from marked health inequality. Employment opportunities and income are some of the lowest in any local authority district in England. The expected healthy life expectancy is 59.2 years for males and 58.8 years for females: 3 years below the national average for each.

Improving wellness, inclusivity and quality of life through integrated care system transformation

Aqua was commissioned in 2017 by the Warrington Health and Wellbeing Board (WHWB) to help co-design and implement a strategy to transform the way health and social care services were delivered in the area, as part of an initiative called Warrington Together.

Partner integration, service transformation and developing leadership capability

Aqua has been supporting Trafford Clinical Commissioning Group (CCG) since 2018, providing expert consultancy and programmes to develop its senior leadership teams and shape change in organisational structure and service delivery.

Quality Improvement in Action: Primary Care Networks
