Healthy Wirral Partnership commissioned Aqua to help them set out an exciting vision to transform how local health and care is delivered, by becoming an Accountable Care Organisation (ACO).
First developed in the United States, ACOs are increasingly being adopted in the UK to bring together providers and commissioners of physical, mental and community health and social care.
It is hoped that this new model of care will help to join up health and care systems and improve care for patients and local communities.
