Personalised Care Learning Collaborative for Long Term Conditions

This mini Learning Collaborative is aimed at a small number of committed teams, working with people with long term conditions, who want to improve their approaches to personalised care.

This 9-month programme will help your team:

  • Understand the need for change in current practice, consider the benefits and challenges of practicing Personalised Care and the tools and resources to support it
  • Work together across and within organisational boundaries to practically apply a range of approaches, tackling challenges associated with embedding personalised care
  • Learn together and apply knowledge of quality improvement, measurement, team building, leadership, sustainability and spread to support this approach.

This programme is accredited by the Personalised Care Institute (PCI) which is accountable for setting the standards for evidence-based training in personalised care in England.

Delegates will receive continuing professional development points on completion of an evaluation of the programme for the PCI.
